ENROLLMENT FORMS AND INFORMATION
Your child must be 5 years old on or before September 15, 2008 in order to enroll in kindergarten for the 2008-2009 school year.
You may download all forms below, complete them at home, and bring them with you when you enroll your child. You will also need to bring your child's birth certificate, social security card, shot record, and proof of physical.
